Valley Transit II
Certification Requirements
Passengers
with Disabilities: must first be issued an ADA Certification Card in
order to use the services of Valley Transit II. A qualifying disability must
prevent the use of the fixed route system for some or all trips, whether it be
physical or mental, permanent or temporary. When possible, the fixed route
system should be used instead of Valley Transit II. Older
Adult passengers:
age 60 or over, living in Outagamie or Calumet County qualify for Valley Transit
II Older Adult Transportation. Outagamie
County residents must use a yellow Outagamie Senior Certification card. The Senior
Certification card can be obtained from Outagamie County department of Aging
Services, 401 S. Elm St., Appleton, WI 54911.
Calumet County residents may use a legal ID, driver's License,
passport, birth certificate, etc. Limited service hours apply. Valley Transit II
Certification Process For ADA
certification, first complete the Request for Certification of ADA Eligibility form. This is
also available at all city, village, and town halls in the Valley Transit service
area and from Occupational Health Systems of Wisconsin, Inc., the Thompson Community
Center.
Next, contact Occupational Health Systems at 920-730-5337 to set up your personal
interview. The personal interview is an important, required part of this certification process.
You will need to take your completed ADA Certification form to Occupational Health Systems at the time of your personal
interview. Occupational Health Systems has two locations to serve your needs.
