For those individuals wishing to be certified to use Valley Transit II, the following form has been provided. This form must be filled out completely, and taken to a required certification interview. To schedule an interview, please contact Affinity Occupational Health Systems of Wisconsin, Inc. at 920/730-5337.

10-Ride Ticket or 30-Day Pass Mail Order Form 

Valley Transit 10-Ride tickets and 30-Day Passes are a great way to save money on the trips you take by bus. For your convenience the following form can be used to purchase your tickets by mail.
