Request for Review of Parking Citation  

A request to review a parking citation applies only when there are circumstances that are unique, extenuating, or beyond the control of the recipient (e.g. meter malfunction, medical emergency, vehicles exempted by statute, etc.).

Tax Payment Timeliness Appeal Form

This form is used to submit a claim to the Tax Payment Board to appeal the Finance Department’s conclusion that your tax payment was not received on a timely basis.

Direct Payment Authorization Form 

Have your City services invoice (water, sewer, etc.) payment made automatically from your checking or savings account.  You will continue to receive a statement showing your utility charges but, on the due date of the invoice, your bank will automatically deduct that amount from your account.
